Qualifications: - High school graduate preferred or GED
- Must be able to read and write well
- On the job training if necessary, but patient care experience is desired
- Good communication and interpersonal skills required
- Must pass strength testing and requirements
- CPR required
Job Relationships: - Responsible to OPS/Endo/Pain Management Nurse Manager, then following the OCH Chain of Command.

Physical Demands: Note: "Occasionally" = 1-33% of the workday; "Frequently"= 34-66%; "Continuously" = 67 -100%. This may also be described as performance once every 3 minutes = "Occasionally"; once every 1.5-3 minutes = "Frequently"; once every 1.5 minutes to continuous work = "Continuously"
- Standing/Walking - Continuous. The Transporters are on their feet the majority of the shift to perform indirect patient care as well as completing other assigned functions of their job.
- Bending - Frequent. Bending is required in most job activities related to patient care. The transporters should be able to maneuver in small areas, such as plugging in electrical equipment and in emergency situations.
- Climbing/Reaching - Occasional. The transporter should be able to climb small ladders, steps or stools when placing supplies on storage shelves, etc. The transporter should be able to reach in order to place supplies and equipment on storage shelves.
- Push/Pull - Frequent. The transporter often performs push/pull motion when transporting patient and moving equipment on various floor surfaces.
- Handling/Lifting 0-100 lbs. - Frequent. The transporter may need to lift and help handle objects from 1-300 lbs. Assistance in moving patients and push/pull activity is frequently done. Due to the physical demand, transporters will be allowed to utilize the Wellness Healthplex.

Both fine and gross motor skills are needed in areas such as vital signs, assisting patients, writing, and adjustment of equipment. Foot control may be used with some equipment.

Patient Population Serves all patient populations when transporting and transferring of patients is needed.
The evaluation is completed by the Nurse Manager. Input from co-workers, direct observation, chart review, and the charge nurse are taken into account to complete the evaluation.

2. During transporting/transferring, recognizes and reports changes in patient condition or untoward reactions to ensure effective and consistent care to patient.

4. Maintains the use and care of supplies and equipment and promptly reports any deficiencies to appropriate personnel. Cleans equipment (wheelchairs/stretchers, etc.) appropriately.

6. Attends mandatory nursing and hospital in-service programs and staff meetings to promote staff development and stay informed of changes in policies and procedures. Maintains CPR certification.

8. Displays a good attitude when transferred to another department to work for the shift.
9. Demonstrates good communication skills both verbal and nonverbal in all situations. Provides hand-off communication utilizing the 5Ps at patient pickup and at patient arrival to units. Promotes infection control measures with appropriate hand washing techniques or using aseptic foam, wearing gloves as needed, observing isolation precautions, cleaning of wheelchairs and stretchers, etc. Knows where to locate all personal protective equipment/supplies and follows standard precautions.
16. Adheres to and understands policies of the area, department, and facility, including but not limited to infection control policies, safety policies, HIPAA policies, disaster preparation policies, bomb threat policies, Code Blue policies, visitor policies, Code Pink, and smoking policies. Is aware of compliance and variance reporting policies.

Specific Duties N/A 12345 Comments 1. Escorts patients to the destination. Greets patient, introduces self, and orients patents and visitors to surroundings including call bell, bathroom, telephone, and thermostat.
2. Verifies patient identify with two patient identifiers. Ensures that every patient has on an armband avoiding operative extremity.
3. Assists and communicates with nursing or ancillary staff with "Hand-Off Communication"
4. Transfer of patient from one surface to surface: a) bed to chair, b) table to table, c) wheelchair to car, d). bed to stretcher, e) stretcher to bed, f) chair to bed, g) wheelchair to bed, h) car to wheelchair safely and to stretcher.
5. Transport of patient from one area to another using wheel chair or stretcher as directed.
6. Assists patients in/out of bed as needed maintaining good body mechanics.
7. Keeps beds in low position. Leaves patient safe and secures bed with side rails evaluated as needed.
8. Receives and gives instructions including precautions (hand-off communication) from department staff in the area requesting assistance prior to performing any patient care or assistance.
9. Assists patients who are bedridden with position changes, reporting any skin breakdown to the nurse.
10. Reports any physical or mental changes in the patient condition to the nurse.
11. Transports patients on discharge. Greeting patient, introducing self, giving driver time to get vehicle without feeling rushed, assisting patient into vehicle including helping with seatbelt if indicated.
12. Assists with preparation of decreased patients prior to removal by funeral homes.
13. Assists with the orientation, training, and guidance of new transporter employees
